Questionnaire privacy notice

This notice explains the information used by the Integritis Complaints Process Check. It should be read alongside the full Integritis Privacy Policy.

Who is responsible for your information?

Integrit Integrated Solutions Ltd, trading as Integritis, is the controller. The company is registered in England and Wales under number 16394610 at Discovery Block 4, Pennine Five, Hawley Street, Sheffield, England, S1 2EA.

What we collect

  • Your multiple-choice questionnaire answers and the outcome band calculated from them.
  • Basic campaign-source labels, such as the email or LinkedIn link that brought you here.
  • If you request the templates or a call: your name, organisation, work email and the support selected.

There is no free-text answer box and we do not ask you to describe a complaint or identify a complainant.

Why we use it

We use your answers to show the result immediately and measure the campaign at an aggregate level. Your result is not conditional on providing contact details.

If you request the templates or a call, we ask for your agreement to use the contact details you provide, your result and your selected support choices to arrange that follow-up.

How long we keep it

Identifying details and individual answers are removed within 60 days. After that, Integritis may retain the outcome band, completion date and support requested for service measurement, without the questionnaire record identifying you.

If you are already an Integritis contact or become a client, your ordinary business contact record may be retained separately under the full Integritis Privacy Policy.

Anonymous campaign results

We keep separate counters for broad answer and outcome totals. These counters do not contain your name, organisation, email address or an answer transcript. Any published statistic should be subject to a minimum sample-size and disclosure review.

Your choices and rights

You can ask Integritis to stop the follow-up or exercise your data-protection rights by contacting dpo@integritis.uk. If you are unhappy with how your information has been handled, you can also complain to the Information Commissioner’s Office.
